When visiting our office for the first time, you will be asked to fill out our new patient intake form to provide the doctor with specific information pertaining to your health concerns. For your convenience, you will find links to download our intake form as well as other insurance related documents here.* Downloading your forms and filling them out in advance will let you take as much time as you need as well as allow you to complete them in the comfort of your own home. Please bring your completed forms with you when you visit our clinic for the first time. If you prefer, you can wait to fill out your paperwork in our office.
*Patients who have MEDICA or United Healthcare as their primary insurance carrier are required to fill out additional paperwork before examination or treatment.

If MEDICA or United Healthcare is your primary
insurance carrier, you will be required to fill out the
"Patient Summary Form" in addition to our new patient
intake form. MEDICA and United Healthcare patients
should also see the Neck and Back Index forms below as
well as the DASH and LEFS forms.
